BASIC EDUCATION

Senior High School
  1. Accomplished SHS Enrollment/Registration Form
  2. Original Grade 10 Report Card / SF9
  3. Certificate of Grade 10 Completion, when applicable
  4. PSA Birth Certificate or acceptable supporting document
  5. Good Moral Certificate, if required by school policy
  6. Two (2) copies of a 2x2 picture
  7. Basic Education Form
BASIC EDUCATION

Senior High School Transferees
  1. Accomplished Enrollment/Registration Form
  2. Original/latest SF9
  3. SF10/Form 137 — to be requested/transmitted between schools as applicable
  4. Good Moral Certificate, if required
  5. PSA Birth Certificate or acceptable supporting document
  6. Previous school credentials
  7. Other school-specific requirements
ALTERNATIVE LEARNING PATHWAYS

SHS: ALS / PEPT / A&E Learners
  1. Appropriate ALS/PEPT/A&E certification or result
  2. PSA Birth Certificate or acceptable supporting document
  3. Other available learner credentials
  4. Accomplished Enrollment/Registration Form
UNDERGRADUATE EDUCATION

Higher Education
  1. Original copy of Senior High School Card (SF9)
  2. Original ALS Certificate of Rating (for ALS graduates)
  3. Original Certificate of Good Moral Character from last school attended
  4. Original copy of PSA Birth Certificate
  5. Photocopy of PSA Certificate of Marriage (for married women; original for verification)
  6. Photocopy of Certificate of Honors, if applicable (original for verification)
  7. Two (2) copies of a 2x2 picture
  8. One (1) long brown envelope
UNDERGRADUATE EDUCATION

College Transferees
